As part of our service, we begin by consulting with the new client and the family members in their residence.
During this consultation, we review your needs through discussions with the involved family members. When necessary, we consult
with the physician(s) of the care recipient, nurses, social workers, hospitals, or nursing home staff.
Next, we work in conjunction with your family to develop a customized home care plan that addresses the client's
specific home care needs. The plan is individualized to manage your daily needs.
Once the care program is developed, we match your needs to the best possible caregivers who we believe are most
suited to the service to be provided and can best meet your needs. You can participate in the selection process on demand.
Our goal is to maintain a consistent caregiver for each client. This allows the caregiver to become very knowledgeable of
the client's needs and helps build an excellent relationship between the client and the caregiver.
After the caregiver has been placed in your home, we continue our system of continued personalized contacts through
monthly home visits and telephone check-in. Whenever necessary, we re-assess your needs and up-date the individualized program.
We continually communicate with the caregiver who is providing care to you and we coordinate the care program with the nursing
home staff, the physicians, and the social workers. We continually monitor your safety and check your progress.

We offer Companions, Home Health Aides, and Skilled Nurses at affordable rates as live-out, live-in,
sleep-over. We provide services from 4 to 24 hours a day, 5 days or 7 days a week, weekends, and holidays. We accept temporay
or long term care plans. The initial assessments of need, evaluations and care programs are performed at no charge for our
clients.
The Reliance Home Care staff is available to answer phone calls, 24 hours a
day, and 7 days a week for all client needs and assistance.
All of our caregivers have years of experience working with older and disabled adults. They have assisted people
with heart disease, stroke, arthritis, diabetes, cancer, Parkinson's, hip fractures and other illnesses. Many of our caregivers
have additional training working with Alzheimer's and other memory loss disorders. Reliance Home Care recognizes the special
needs of these clients and their families and plan appropritate care and activities especially for them. Some of our caregivers
have had training and experience in caring for terminally ill people at home. They are adept at respecting the special wishes
